She has had asthma since childhood, was intubated once three years ago, and has used four salbutamol inhalers in the past month with no controller therapy. She is agitated, unable to complete sentences, respiratory rate 34, heart rate 138, oxygen saturation 89% on 6 L/min, with a silent chest on auscultation; the first arterial blood gas shows pH 7.28, PaCO2 6.9 kPa, PaO2 7.4 kPa, lactate 4.1. How would you assess and manage this patient?
What the panel is assessing
The panel is testing whether you can recognise near-fatal asthma instantly, escalate without waiting for investigations, and then convert a life-threatening event into a structured secondary-prevention plan. They also want to see that you understand the physiology of ventilating a severely obstructed patient.
