You are the on-call consultant physician covering a 60-bed district hospital without a catheterisation laboratory, three hours by road from the nearest primary PCI centre. A 58-year-old taxi driver arrives 90 minutes after the onset of crushing chest pain with ST elevation in leads II, III and aVF and reciprocal depression in aVL. He is alert but clammy, blood pressure 82/54 mmHg, heart rate 48 per minute, jugular venous pressure elevated and the lung fields are clear on auscultation and on portable chest radiograph. The emergency registrar has already given sublingual nitrate and 40 mg of intravenous furosemide and is asking you whether to start noradrenaline. How would you manage this patient?

The panel is testing whether the candidate recognises right ventricular infarction as a preload-dependent state and can construct a reperfusion strategy in a resource-limited hospital where transfer time exceeds guideline-recommended PCI-related delay.
