He is cold and clammy, blood pressure 74/40 mmHg, heart rate 128 and irregular, respiratory rate 32, saturations 90 percent on a non-rebreather mask, and lactate 5.8 mmol/L. He is drowsy but rousable, and his jugular venous pressure looks elevated. You are the consultant on the floor with one resident, two nurses and a full waiting room. How would you approach this patient?
What the panel is assessing
The panel is testing whether the candidate can run a structured resuscitation of undifferentiated shock, using bedside ultrasound to narrow the differential rather than reflexively giving fluid, while leading a small team under load.
