A 24-year-old Emirati university student is brought to your emergency department at 9 pm by his father and uncle after two months of withdrawal, reversed sleep and the belief that neighbours are broadcasting his thoughts. For the last three days he has refused all food prepared at home because he believes it is contaminated, and last night he pushed his mother out of his room while shouting at an unseen voice. He is guarded, thought-blocked, mildly dehydrated and has no previous psychiatric contact; the family are adamant this is sihr or jinn possession, refuse admission and want to take him to a well-known raqi first. How would you assess and manage this patient, and what would you do if the family attempt to leave with him?
What the panel is assessing
The panel is testing whether you can hold acute risk, an organic differential and a first-episode psychosis formulation together while negotiating respectfully with a family whose explanatory model differs from yours. They also want to see that you know the involuntary pathway under UAE Federal Law No. 10 of 2019 but do not reach for it reflexively.
