You are the consultant on call in a private tertiary hospital. A 34-year-old para 3 has just had an emergency caesarean at 39 weeks for failure to progress after a long augmented labour. Twenty minutes after closure the recovery nurse calls you: the uterus is boggy, the drapes and floor are soaked, measured loss is already 1800 mL, blood pressure is 82/44 mmHg with a pulse of 132, and she has had oxytocin bolus and infusion plus one dose of ergometrine with no response. How would you manage this patient?
What the panel is assessing
Whether the candidate can lead a time-critical resuscitation with parallel medical, mechanical and surgical escalation rather than a serial checklist, and whether they know when to abandon uterine conservation. The panel is also listening for team leadership and quantitative rather than visual blood-loss assessment.
