You are the on-call consultant neurologist at a tertiary hospital at 02:00. A 71-year-old man with atrial fibrillation is brought by ambulance 95 minutes after sudden onset of global aphasia and right hemiplegia; NIHSS is 19. His wife says he takes apixaban and, after checking her phone, believes the last dose was at about 21:00 the previous evening. Blood pressure is 178/96, glucose 7.4 mmol/L, and non-contrast CT shows a hyperdense left middle cerebral artery with ASPECTS 9. The emergency physician has drawn up tenecteplase and is asking you to authorise it. How would you manage this patient?

Whether the candidate can make a rapid, evidence-based thrombolysis eligibility decision in an anticoagulated patient without losing time to reperfusion, and whether they understand that thrombectomy is the definitive pathway for a large-vessel occlusion.
