Why: Suspected alteration of a controlled prescription requires the document to be retained, the pharmacist informed, and the prescriber contacted to verify the intended quantity, with escalation to the authority as required; direct confrontation risks staff safety and may destroy evidence. Partial dispensing without verification, public accusation, and dispensing the altered quantity are all inappropriate responses.
